ICE Card Instructions

Click on the link to bring up the downloadable form.  Then click the download icon to download the form to your computer, save the form on your computer and begin completing electronically. Use the icon that looks like an arrow going down into a box.

 There are two versions of the form.  Version 1 simply repeats all information on the front and back of the card.  Version 2 provides you with an entirely blank back to allow you more room to enter additional information if you think the box on the front is not sufficient.

The form is a fill-in form.  Simply position your cursor at the beginning of each field and begin typing the applicable information.  The form is formatted to allow you to type as many characters as you need, the text size will adjust accordingly as you type.  You can also complete the form by hand, but typing is preferable to prevent any legibility issues.

The DOB and the Number fields are auto-formatted as date and phone numbers, respectively.  Typing slashes or dashes in the DOB is unnecessary; the form will convert what you type into a Month/Day/Year format.  Similarly, typing dashes or dots when you enter a phone number is unnecessary; the form will convert what you type into a (area code) ###-#### format.

Please be sure to include both a primary and secondary contact.  As for the rest of the information, think about what a first responder might need to know prior to treatment and enter whatever you see fit.  

The organ donation box is optional; just enter “Yes” if your wish is to be a donor or leave the box blank.

After completing the form, you’ll need to print it (in color if you can, but black and white is okay).  The form is set to print in a size that will fit in your wallet like a credit card or driver’s license.  In order for the form to print properly, you must disable the document scaling in your printer settings.  This may look different depending on your computer and printer, but below is an example of the setting you are looking for and how it should be set.

After printing, cut the card out and fold in half, or cut the two halves.  Clear plastic sleeves to hold and protect the card will be available at the 2021 Kick-off Breakfast or anytime in the service area at Bair’s.
